DHSS pilots transition scheme

A scheme helping older people make the transition from hospital to home is being trialed in the south of the Island.

The new Department of Health and Social Security service will be based at the Bradda Unit at Southlands Resource Centre in Port St Mary.

People over the age of 65 who meet certain criteria, will be given a personal plan before moving into the Bradda Unit for a brief period.

They will then move home with continued support from the health services.
